meaning that it is not just courtesy for a few..but for everyone.
probably british in origin..meaning that even the common people..the lower class deserved courtesy.

To me, common courtesy means common sense of when to be nice to someone, like opening doors and things like that.

Common as in - available to all. i..e. common ground.
Common courtesy can be set aside from say - eitquette - which is perhaps best described as fashionable manners and therefore many and complicated.
Common manners include the 'thank yous' and the 'please's' of everyday social intercourse.
It has nothing to do with the user - or abuser - of manners being 'common'!
