he is pretty big

he has one big hole in his head like the size of a pencil eraser
a light red in the middle surrounded by white
looks like there is smaller holes on his head also although these are just holes and much smaller.
i am very concerned, any ideas on what this is???

Sounds like Hole-In-The-head (HITH) disease to me, a disease that's commonly believed to be caused by the protozoa Hexamita. There are many different causes, however the most common are inadequate housing, poor water conditions and/or some form of nutrient deficiency will weaken the fish's immune system and then the hexamita moves in.

Unfortunately, I've heard it's rather hard to treat due to the many contributing causes to the disease, and it often requires a lot of attention to details and water chemistry. However, most web pages will recommend metronidazole (Metro+ by Aquarium Solutions) for treatment, as well as religiously keeping tabs on water quality (Ammonia 0, Nitrite 0, NitrAte less than 10). Also, improved and well-rounded nutrition is a must.

Well hole-in-head disease isn't rare, it's quite common in Oscars and sometimes in other cichlids. Jungle brand makes a hole-in-head treatment. Sounds like he's had it for awhile...get the treatment, if possible separate him but you probably want to treat the whole tank anyway.

This is a very rare disease called hole-in-the-head. Separate him immediately.
