Fleas and Rats.
The rats travel around the world in sailing ships, got on and off ships as they travelled and spread the plague.

the black plague, also the bubonic plague was i think, spread in the bite of a rat, and spread even farther because living conditions were so condensed in the middle ages, this along with terrible sanitation contributed to the contagious and deadly disease. and the bodies kept piling up and they didnt even take the time to bury them which was another health hazard.
