Before I try to answer the question with known facts, I can take a guess at races where no-one appears quite sure how many times the lead changed, as it often happened several times during a lap. The 1970 Italian GP at Monza is one such race, with Ickx, Stewart, Reggazzoni, Rodriguez and Oliver changing the lead several times up to lap 13 when Rodrigues retired. Oliver then led for a while on his own before both he and Ickx went out to mechanical troubles and left Stewart and Reggazzoni to swap the lead a few more times before Regga finally pulled away to win by 6 seconds...2nd-5th were only covered by 0.68 seconds! Also, the Indy 500 counted towards the F1 World Championship from 1950-1959 and I suspect any one of these races could be the real answer to the question as Indy tends to produce dozens of lead changes with all the pit stops.
Monaco 1982 had several lead changes, including 4 different leaders on the last lap. Arnoux led but spun, allowing Prost to take over. He led the entire race until the 74th lap, when the engine packed up, allowing Patrese through to lead the penultimate lap. He had a spin and dropped to 4th, and Pironi led into the final lap before running out of fuel, allowing De Cesaris into the lead. He then also ran out of fuel, which put Daly in front. He then had an accident, allowing Patrese back through to claim victory!
The best answer I can come up with however is the 1967 Italian GP at Monza. Here are the consecutive leaders (in a race without refuelling):
Brabham- Gurney- Clark- Hulme- Clark- Hill- Brabham- Hill- Hulme- Hill- Brabham- Clark- Surtees!
So that makes 12 changes of lead without refuelling stops, it's the most I've been able to come up with that can be actually verified by various reports. The first three on the list were the leaders at the end of the 1st, 2nd and 3rd laps respectively!

Not quite sure about that. Usually there will be about 3-different leaders in a race. Most leads? Probably at Spa. it's a crazy race there esp if it rains.

For NASCAR:
Talladega: Holds the record for most lead changes in a NASCAR Cup Series event 75, May 6, 1984
Daytona: 60 lead changes in 1974
more current is Texas Motor Speedway 29 lead changes in 2000

For IRL: I think???
