Yes, We normaly charge 200 dollars to pump and inspect the tank. If you have the top dug out it is still 200 dollars. Digging it out is all part of the job so yes it is very normal to charge for this. We even have customers that have an extra charge if thier tank is very deep or hard to acess

When I had a septic tank (was just recently forced to switch to sewers) they always asked that we dig it up in advance, they took the lid off and put it back on though. I've never heard of a company digging it up for you, so I'd imagine that they would charge, I know I sure as hell would if I were them.

definately, most companies dont have that cost broken out though it is just figured into the fee.
