I hear a grinding noise from the back right side of my 2003 Honda Element. I definately has something to do with the rotaion of the wheels; it changes as the speed increases or decreases. Here is what really concerns me: when the brakes are applied, the grinding noise is worse. Obviously, it needs attention but I'm wonder what I am looking at here in terms of repairs.

Sounds like you've gone beyond needing a basic brake job to needing a new drum or rotor. What you're hearing is probably metal grinding on metal because you've worn out the brake pads or shoes. You've just escalated the cost of this repair by $150-200, maybe more. Go get it checked now, because it will become a safety issue very soon.

Listen to the first answer, brakes are probably the problem.

If brakes are bad, there is obviously a noise due to metal to metal contact.

Also, if disc brakes, you may need to replace the brakes and rotor.
If drum style, you may need to replace the brakes and drum.

(note:) Sometimes it is possible to "turn" the rotor/drum is wear isn't to bad, something to consider when you do repairs, may keep cost down instead of replacing.

You need brakes. If you go to one of the service chains, you will probably pay $80.00 or so. Maybe a little cheaper if you go for the rebates that some offer these days. The price will be per axle (both rear OR both front). They may tell you to replace the rotors which will be extra. If it hasn't been grinding too long you shouldn't need new rotors on that car.

Have you had the rear brakes looked at?

Could possibly be brakes going metal to metal on a drum.
