you can get a copy of your birth certificate at your city's vital record.

As for you Social Security Card, it will take 2 weeks for it to come in the mail. You will need an id (passport, school id, insurance card, driver's license, etc) to get it processed (and it must be original). Make sure you give them your current address (include apt #). Make sure the service rep. gives you a receipt, as proof you filled out the application (some service reps are sneaky like that).

You can either go on-line to get an application at www.ssa.gov or you can call them at 1-800-772-1213.

Replacement SS cards usually take 4-8 weeks, and in order to get a copy of your BC you need to contact your state's dept of health/vital statistics and find out the nearest location.
