off meaning insane? What does that really mean

When someone asks that it usually means you are acting differently then you usually do. My buds usually ask me that when I am upset or something happens where I've been up all night or something like that.

I think when they say that you seem a little off they mean that your coloring isn't normal- perhaps your cheeks are flushed or you are pale. I always take it to mean that I don't look like I normally do when I'm well.

It can also mean that the persons demeanor/attitude isn't as usual, maybe they are crabby or irritated, could also mean that they are having a bad day. (One of those days where nothing is going right and you're a little "off") You're off your mark- meaning off from where you usually are.

It means the person in concerned about you and checking to be sure you're all right. Saying you seem a little off isn't necessarily saying you're insane, it means you're not acting as you normally do.
