There is usually a mutual agreement between the couple. Couples who don't use protection usually agree to be "exclusive" (which if they're married I don't see why they wouldn't be exclusive in the first place).
Many couples still choose to use protection though to prevent bacterial infections such as urethritis which can lead to a bladder infection if left untreated. Anal douching is also commonly used for hygenic purposes and to decrease the risk of bacterial infection (although this practice can increase the risk of STD infection by washing away the mucous membrane in the anus)

Gay men who are trustworthily monogamous and have been cleared for any risk for STD transmission may or may not use condoms, depending on their own taste. You may recall that HIV was spread because condom use was not entirely common prior to the advent of AIDS. You may gather from that that the concerns about cleanliness was managed in different ways when condoms were not employed.

No, my partner are monogamous and we don't use protection. There are measures in which one can take to avoid ... well, you know.
But, that's just our preference.
