okay, I dont know what to put in the blanks for this form for school...
#1) I am the parent/guardian (circle that which applies) of ________________ (names of minor student) (hereafter "the Minor")
#2) in the presence of: _______________(witness)
who is my witness??'

1st. if you are the kid's parent, circle parent. if not, circle guardian.
2nd. write the kid's name.
3rd. find someone that is at least 18, and write their name and have them sign it.

In the first blank, put your child's name.
For the second blank, get a friend over 21 to witness you signing the form. Put that friends name in the blank space.

Fill in the name of the child
#2 Whomever is with you and is over 18 can sign as a witness.

#1 either you are the parent (gave birth to the child or are the biological father) or guardian (legal papers) of put the kids names in.
#2 whoever watches you sign the paper is your witness.

Generally a notary public can act as a witness for a legal document, they can also notarize that you are the signer and the date/time of the signature. That's your best bet.
