Any time that a ball is mishandled to the extent that the batter-runner was able to reach base or that any player was allowed to advance when that player ought to have been out, the official scorer decides whether the play is a hit, a fielder's choice, an error or a combination of both a hit and an error.

The e in the box score stands for error. An error occurs when a player has a chance to field a ball but does not. An example of an error would be when the shortstop fields the ball and throws it to the first baseman but the ball goes sailing over the first basemans head.

In baseball, an error is judged by the official scorer when a runner advances a base because of a fielding mistake, and perfect play would have prevented the advancement, and the mistake was physical. Mental misjudgments are not errors. Failing to get more than one out on a given play is not an error. Application of this rule is necessarily subjective.
