The person writing the check determines this, not the law. They can stop payment of the missing check and reissue it immediately or wait until their next bank statement.

Not long but you need to contact the source of the check-they will put a stop and trace order on the check in question-sometimes you will get another check to replace the lost one during this process-depending on their policy regarding this issue.

Depends on who issued the check. If it was the govt...a long time. Payroll, probably depends on the state. Unfortunately, this is probably not much help.

I would contact whoever issued it and ask them their policy. I would guess that in most cases, it is shorter than the law allows.
