The black thing you're seeing is most likely the hemipenes. Your turtle might be constipated, or it might have eaten a piece of gravel if you use it in the tank. Turtles get a little enthusiastic when eating and don't always swallow the right thing. If it's small enough, a stone may pass without too much of a problem. He might also have an intestinal parasite or infection which could give him a problem.

For now, I'd say make sure his tank and water are clean and make sure he's got the proper temperature conditions. You don't say how long this has been occurring, but if it doesn't resolve in a few days, or appears to be getting worse, I'd make an appointment with a vet that is knowledgeable about turtles. I'm not sure if this happens to turtles but I don't see why it couldn't, it could have prolapsed, which is when inner parts decend and fall out of place.

Also many turtles use the the cloaca for respiration when they are underwater, so it could be something related to that.
