My Red earred slider turtle's Cloaca appears to be swollen and when held a black thing comes out and back in

the tail appears that it may be slightly swollen, could he be constipated? or is this something else?

The black thing you're seeing is most likely the hemipenes. Your turtle might be constipated, or it might have eaten a piece of gravel if you use it in the tank. Turtles get a little enthusiastic when eating and don't always swallow the right thing. If it's small enough, a stone may pass without too much of a problem. He might also have an intestinal parasite or infection which could give him a problem.

For now, I'd say make sure his tank and water are clean and make sure he's got the proper temperature conditions. I'm not sure if this happens to turtles but I don't see why it couldn't, it could have prolapsed, which is when inner parts decend and fall out of place.

Also many turtles use the the cloaca for respiration when they are underwater, so it could be something related to that.

The black thing is probably the hemipenes.
if it is swollen it might be constipated or an allergic reaction.
