If I want to remover shower knobs, spout, etc, so I can tile the tub surround, is it necessary to shut off the water at the meter? I removed the cap and took the screw out, but now what do I do? Does the knob just pull straight out?

The handles should come straight off. They may be a bit sticky but they will come off. If all you do is take the handles off you shouldn't have to turn the water off. But while you have it apart it would be a good time to replace the washers in the faucets, now you will have to turn off the water.

The handles usually just pull straight off. Problem that happens is calcium deposits build up on the stem and "freeze" up the stem to handle. You can try wrapping a rag soaked in vinegar around the handles for an hour or so. Otherwise you will need a handle puller. Almost like a bearing puller. Tub spout will either screw on or have an allen screw in the bottom inside a slot near where the spout meets the wall (underneath). Remove the screw and pull the spout off. No need to turn the water off as long as you are carefull not to turn the stem on the handles while you are working.

That depends on if you have a shut off valve before teh shower/tub. If you do then no you don't have to if you don';t then you have to shut off at the meter. Both the stems and the spout unscrew; however for my stems I had to get a special tool from Home Depot (~$6). Good luck tiling can be such a total pain in the buttttttttttttt

did you come out the middle cap w-a screwdriver and notice if there's a screw in there conserving it on, get rid of the screw and then faucet on it to loosen it up somewhat even as attempting to tug it off.
