You could make it yourself if you have a CO2 tank, a pressure vessel, and a means to refrigerate to very cold temperatures.

Otherwise, check the yellow pages under "ice".

Some stores might give it away for free, like ice cream shops. Ask around.
As for making it yourself, it would be pretty difficult. Just finding carbon dioxide that you can manipulate is at least as hard as getting dry ice that is ready to go.

You can probably buy it at your local grocery store. You'll have to be 18+ though.

You could make it if you had a freezer capable of reaching -72C, but you'd probably spend several times more on electricity making your own than buying a 25lb bag.

if you are over 18 then just look in the phone book. you can make it but it isn't worth the time or expense unless you need a constant supply

usually butcher shops have dry ice for sale. check your local listings to see if you have one nearby
