It simply means if you remove all the diamonds (stones) in a particular piece of jewelry and place them on a scale, the total weight would equal...
For ex: Let's say you have a 5 stone ring that's advertised as a "3 CTTW diamond ring". If you remove all 5 stones and place them on the scale, the "CTTW" will equal 3... So, all 5 stones in said ring (when weighed ) equals a 3 CTTW diamond ring.
