Remove the grounds from the can, put them in your hand and gently shake around the base of the bush, rake in to about a 1-2" depth

yes mix in with a small shovel chop up the soil around it but,
Heres a great idea make an area in the back of your yard with some sunlight and mix old grasses, leaves, coffee grounds, banana peels, egg shells and anything else organic ( stay away from full foods because bugs and animals) together to get compost. In the meantime you can mix up some of the coffee grounds but in reality it will not be as useful as the compost, mix both into the ground. Your compost will take a year or so to breakdown, much more info about compost piles but do a quick search and you will find more hlpful info.

You are talking about the used left-overs I assume. Their main use is as a soil conditioner, not a fertilizer. Sprinkle them over the area around the plant and stir them into the surface to enhance the soil texture there.
