I need to know ASAP, the case worker isn't calling me back.

No forms you send him will take him to court. If the caseworker is not returning your calls, call and ask to speak to the supervisor. If that doesn't work, call the state office of child support, if they are not responsive, call your governor's office.

The court administrator usually tells you which forms need to be sent.If they havent i would call them and insist on some information so you can get things going..

Have your local child protective service do all this for you. They will administer everything and take him to court too if necessary.
