can someone help me with email/html?

I'm trying to figure out how to put a box on my myspace page where people can enter their email to sign up for my email list to get monthly emails each month. You mostly see this type of thing on artist pages on myspace. if you dont know what im talking about leave it here and ill add in a link to tell you what im talking about.

All you need to do is to create a simple mailto form.
