I asked myself that question around the age of 11 or 12. Although from an age even younger than that I did not identify well with my birth gender. I know for a fact that people can be confused about their gender. Although I never really was "confused" about what I felt, I'm sure people who are not educated on these types of things will be confused about it if they happen to experience.
It's more of something you can feel from as soon as you can comprehend gender differences, sexuality, and all of that stuff. You just know when you don't feel comfortable with what you're known as.

First of all, a little clarity-
Sex is physical, the stuff between your legs, whether you are male or female.
Gender is identity, the identity you have of yourself, and it's between the ears.
I knew I was a girl from very young- like 3 or 4. I never questioned my gender (identity), I just thught my sex was all wrong- I wondered why people kept referring to me as a boy because I had boy parts between my legs. I thought everybody else had issues about this- for me, it was not a question of who I was- I was a girl inside.
My gender was female, but my sex was male.
Now, some people question their gender because they felt neither completely male or female, so it leaves them in a unique predicament- which side do I identify with more, male or female?

From what I hear from the different shows on the topic, seems most were like that from the start, and identified as the opposite sex from a very early age.

I think they can and alot are. Just read the postings here, and you will see that. Some just are transgendered and you'd have to ask them,I think the age they felt that way might most likely differ from person to person.

yes, of course they can, just as people get confused as to their sexuality etc etc.
not sure what age, but i've seen a newspaper report about a kid at the age of 10 getting sex reassignment surgery.. it depends on the person, but it can start off pretty young.

According to the interviews on the ABC News special on transgendered children, they can be aware of being in the "wrong" body when they're toddlers.
