There is NO SUCH THING as a walk off grand slam. A grand slam occurs with the bases loaded and the batter homers. If a batter walks with the bases loaded, a FORCED RUN scores.

The first poster is a moron who doesn't even know what walk off means.
The answer is no, however. See the following link for all of the walk-offs in WS history. Maz in 60 did it with the game tied 9-9, 0 outs, 0 on, on a 1-0 pitch. It won the series in the final game (game 7).
Carter in 93 did it with his team down 6-5, 2 outs, 2 on, 2-2 pitch. It was game 6, so had he gotten out they could have played the next game still.
Hoiles for the Orioles hit the "perfect" grand slam (full count, bases loaded, 2 outs, bottom of the 9th, down by 3 runs) but it was in regular season.
Clemente is the only player with a walk off inside the park grand slam.
*EDIT* I just realized this question only asks if anyone has ever hit one. A few have done it in the scenario you mentioned (I think they are all listed in my link), but only one has ever done the "perfect" one, which I mentioned above.

There have been several occassions when a walk-off grand slam has won the game, but I'll do you one better - I was at a game in 1996 when Orioles catcher Chris Hoiles was up in the bottom of the ninth, bases loaded and his team down by three. He hit a full count fastball over the left field wall and his team won by a run.

I was at a minor league game and witnessed a walk-off grand slam. 2001, I think. The fans went nuts. I was up and ready to leave -- glad I didn't.
