Condoms are over 95% effective, but in reality are used effectively only 75% of the time.
It depend on whether there is any penis/vagina contact before the condom goes on. It is no good putting one on half way through - a man can leak up to 6 million sperm in pre-***.
If anything oily comes into contact with the condom it can make it burst - like vaseline, baby oil - things you might use as lubricant.
If the condom does not have a quality mark on it - like a kite mark or CE then again it could burst becuase it is not a good quality condom.
They can also go out of date and the latex can perish if stored too hot or cold!
However if you get your condoms from a trusted source and use properly the risk of pregnancy is very low.

I really depends on whether or not the users actually use them correctly. The most common causes of failures are slippage and breakage. Slippage can be avoided by the person wearing the correct size of condom. Breakage can be avoided by using new condoms that have not been left in a hot place for too long. Never use petroleum jelly as a lubricant because it also weakens the condom. Another way to increase their effectiveness is to use them along with a spermicide. The typical use rate of effectiveness is 85%. With perfect use it is 95%.

I don't trust condoms. One time I was having sex with my boyfriend "with" protection and when we were done...we couldn't find the condom anywhere. It's very strange because we don't know where it went. Even til the day we moved out still haven't found that missing condom that we were using. And I got pregnant.

It's much easier using a female (if you're male) or vice versa.
Never seen a pregnant condom, but guess it could happen.
May be a bit risky turning the used ones inside out!
Seriously though, read the literature.. there's a risk to everything

If used correctly and every time, condoms are 97% effective. If you have sex, then there is always a chance of pregnancy.

I think many of you are fooling yourself. Your idea of 1% possibility is ridiculous. The possibilty is as high as with anything else. I wouldn't trust a condom for 1 second to prevent pregnancy.
