95 Jeep Grand Cherokee. V8. 180k miles. The vehicle drives fine in the city and runs smoothly. When I'm in top gear on the highway it sort of shudders and misses. I can feel a little clunking every few seconds, especially when I try to accelerate or go up a hill at speeds above 55mph. Is this something I could fix on my own in a weekend? The most advanced things I have done are brake/rotors, changing water pumps, etc.

that doesn't sound like transmission...that sounds like you need a tune up...the missing and hesitation is happening under a load condition... it's tune up time

i think your problem is your catalstic converter on your exhaust,sounds like its plugged,ifyour going up hill and its losing power ,have that checked out first.
