Step 1: Get the marriage registered with the marriage registrar of the place where the marriage was solemnized. This registration can be done under section 8 of the Hindu Marriage Act, 1955 for Hindu form of marriage, or under the Special Marriage Act, 1954 for any form of marriage. Step 2: In case of change of name, applicant should insert advertisements in two reputed newspapers (one local newspaper of the area in which he/she is residing and 2nd in newspaper of the area of permanent address) Step 3: Incase you want to change the name or surname & add the name of your spouse in the passport a woman applying for change of name/surname in existing passport due to marriage must furnish (i) Photocopy of the Husband’s passport, if any, and
(ii) An attested copy of marriage certificate issued by Registrar of Marriage.

It gets changed automatically. Only notify the appropriate departments (concerning ration office, driving licence, EPIC, passport, bank pass book, the place where you work etc.) that you have changed your surname after marriage so that they can bring it into effect. Enclose a photocopy of your marriage certificate with your representations.

for this u have to hire a lawyer and have an affidefit done before the magistrate/notary public. for this u have to produce ur age proof,marriage proofs (in form of marriage photos). once the affidefit is done u have to flash it in atleast one leading local daily news paper in a proper format which the lawer will advice you.
