I am with United Health care.

BUT, different insurance companies and plans are different.

After your consultation and they take pictures and send in the forms, wait a week and call up your insurance company and ensure they have received the documentation. Two weeks later, call them again and see if they've come to a decision.

It took me 7 months because my insurance wasn't being very nice. I can finally get my reduction in about a month.
