Depending on your State and County, the Court has a website that you can download these forms for free. It will also tell you in detailed of how and where you need to take the complaint as well as the fees involved "if any".

You would be better advised to go to your local courthouse, handling probate matters, and get the forms there. Each state, and sometimes different counties, have their own versions. Most of them are pretty similar, but they don't like generic ones.

every county may have their own type form. See if your county has a website to get them from or go to the clerk of courts office. They should have any kind of forms one might need.
