What kind of information will one find in a patient's file?

For a project, I am making a mock patient file's for certain characters in a story, I was just wondering what kind of info is stored into these?

[and i know this is in the wrong sub. but i know there are a million people in this seciton]

not neccessarily in this order

Name
Birthdate, and Birthplace
Physical descrpition of patient, including identifying markings
List of family ailments that may be inheritable or may show up later in life in the patient (heart disease, cancer, diabetes, etc)
Psychologial evaluation, if the patient has ever had one done
Dental records
Drinking/Smoking/Drug habits if known
List of previous surgeries or procedures, as well as previous medications
List of current medications
List of allergies, medicine/food/and environmental
List of previous ailments and complaints, along with a date of their corresponding visit. Serious Illnesses are usually marked somehow to make them stand out; the method of marking differs, but they may be on a differant color of paper, or have a colored marker in the corner.
Shot/Vaccination Record
Brief Travel Records, just in case they travel to a place that has an endemic or the such. Older travel records may be thrown out if the file is too cluttered.
Criminal record, so the doctor knows if the patient is prone to drug pushing, doctor shopping, or drug abuse.
List of other known doctors/hospitals the patient has seen.
LIst of other known doctors/hospitals the patient is currently seeing.

The format of these papers differs from doctor to doctor; many of them are on check list types of forms (medications and family ailments most often are) and some of them are written up on plane paper.
