I like to go to the court house and they can help you fill them out... I also go to the family justest place to get help with any paper work!!!!

www.courtinfo.ca.gov - Go to FORMS. Select Family Law. Scroll through the list to find the form(s) you need. If you don't live in California, contact your local courthouse for their website, or visit them to obtain the forms. Some stationery stores sell legal forms, also.
Hint: On the form(s), the legal code is referenced. For instructions and information on how to complete the form(s) visit your local Law Library (usually at the courthouses) and read the codes. Some courts require you call them to obtain a hearing date, others assign the date when you file the forms. The Clerks of the Court cannot give you legal advice (how to complete the form - what to put on the form - which form you need). They can tell you what to do with them once they are completed, such as filing the original and having copies for conforming, serving the other party with a conformed copy so many days before the hearing, keeping a copy for yourself.

Most states have their own website. You can download from that website. They also have numbers you can call on the website.
