Exactly how is it determined what two teams play at the world series? Is it based on win / loss % in each league?

Eight teams advance to the postseason.

The six division champions, and the second-place team with the best record per league, occupy the eight berths. Various tie-breaking methods exist if needed, some requiring additional games played. (There has not yet been more than two teams tied that required a game tiebreaker.) In the event that a tie needs to be broken only to determine seeding, and not if a team will go or stay home, non-play methods are used (head-to-head record, that sort of thing).

The three division champs per league are seeded 1-2-3 by overall record. The wild card gets the 4 seed no matter what.

In the Division Series (best 3-of-5), #4 plays #1 and #3 plays #2 -- higher seed gets homefield advantage (games 1, 2, and 5) UNLESS the wild card is from the same division as the #1. In that case, the matchups are 1-3 and 2-4.

Division Series winners advance to the League Championship Series (best 4-of-7). Higher seed again gets homefield advantage (games 1, 2, 6, 7).

LCS winners advance to the World Series as league champions. Homefield (games 1, 2, 6, 7 again) is determined by which league won the All-Star Game earlier in the season (not a very good method, but there it is).

Winner is the season champion.

And then there's no baseball for several months... :(

There are 8 teams that make the playoffs. The winner of each of the 6 divisions which is determined by the team with the most wins. The other 2 are wild cards, which is the team with the most wins to not win their division. There are two rounds of playoffs in the American and National League. the teams that win both rounds meet in the world series

4 teams from each league make the playoffs. 3 from each league win their respective division and 1 wild card winner from each league. The wild card team plays the team with the best record unless they are in the same division. For example if the Yankees win the AL East and the Red Sox win the Wild Card and the Yankees have the best record in the AL then the Red Sox will play the team with the second best record. Then to get to the World Series the winner of the AL plays the winner of the NL.

the winner of each of the three division get automatic playoff spots. the team with the best record in their league get the fourth spot called the wild card. the team with the best record plays the the team with the worst record in the first round ALDS/NLDS. the two other teams in the middle play each other. this series is best of seven. the 2 winners from each league in the DS play each other in the ALCS/NLCS(best of seven). these final two teams become the NL/AL champs and play best of seven in the world series.
last year the world series champions st. louis cards had the worst record in all of the playoff teams and the worst record for any world series champion ever.
