I would like to make him a copy of a slideshow with audio background. Will he be able to read it on his PC and on his DVD player. He lives overseas in Italy. Which format is the safest to go with? DVD + or - ? Is there a difference?

No matter + or -
The only real problem could be a specific player (both PC player or stand-alone DVD player) may not "like" a specific DVD brand, but that's a worldwide issue, the risk is the same for every player in the US as well. Burn it using a slow speed in order to reduce this risk.

PAL / NTSC issue... is not an issue, every DVD player can playback NTSC on PAL TV/monitor.

On the other hand, be sure not to set a regional coding.

Yes it will, use DVD- , its a much safer option
