It's not possible to make a form with email delivery without a CGI. There has to be something on the server to take the form data and send it as an email. HTML alone can't do this.
The only thing you can do with HTML is create a mailto link. You would do exactly the same as if you were creating a normal hyperlink to a web page, but instead of http://www.whatever you would put mailto:email@address.com as the destination. The user can click on this and it would open up their email program, with the address pre-filled. However note that it relies on them having an email address and a configured email program, and wouldn't work for anyone using a webmail service like GMail or Hotmail.
