It depends on whether your GP is prepared to write one. Private medical reports are generally in the region of about £75-£100. However, if you see a private doctor, they could charge more or less what they want.
There is no need to get a private sick note. If you are ill, you are ill and your NHS doctor will certify you as such.

My gp's surgery has a board at reception with prices of sick notes, vaccinations etc, so you can see before deciding what to do.

GP's typically charge anything from £7.50 to £15.

Ask the Receptionist at the Surgery (she/he might do it for free)
