Most time's you'll need a pencil, or an already filled resume. You'll need to fill out information on your theatre/film backround, and what part(s) your auditioning for.

They usually ask you what other show's you've been in, and questions about why you chose their show to audition for.
Usually you need a monologue and a 16 bar song to perform.

They might also have you do a cold reading process- which is where your given a scene from a show, and told to perform it to the best of your abilities. You might also do games with the group, like improvisation games, for them to see how well you can think of something at the top of your head.
They'll want to see the transformation from your self to your stage self; and how the stage self may fit it's self into the character.

I hope it helps ya!!!

It depends on whether it is a school, community, or professional theatre company. In general, it would be wise to prepare 16 bars of an upbeat showtune (bring the sheet music with you) and memorize/prepare a 2-minute monologue (might want to learn a few so you can pick one that fits the style of the show). Bring an 8x10 headshot photo (don't expect to get it back and make sure it actually looks like you), and attach your resume of acting experience and training to it (trimmed & stapled back to back). If it is for a musical, wear clothes that you can move in that will make you easy to spot and remember, but that DON'T hide your body. You might be over-prepared with all that stuff, but you definitely won't be under-prepared.

Be sure to state your name very clearly as soon as you begin your audition. You COULD be asked to sing your prepared song, learn part of a new song, recite the monologue, learn dance steps, read parts from the script, whistle, yodel, do a cartwheel, or any number of other things that are part of a role for which you might be considered. You aren't expected to totally understand the characters and plot for a cold reading, but it would be wise to read and/or learn about the production ahead of time, since it might give you an edge over others. It doesn't hurt to do a little research on the company and/or the Director, so you'll know a bit about whom you're dealing with.

Most importantly: PROJECT and ENUNCIATE and if you mess up, DON'T apologize or make a big deal about it - GO ON, just like you would if something like that happened during a performance. DON'T copy the movie, if there is one. DO try to do anything the Director asks you to do - and don't worry about looking foolish - remember - he is trying to see if you can be directed AND if you can meet HIS vision of a character.

Keep in mind that you aren't really competing against other actors - you're competing against your own abilities/limitations AND the Director's idea of the character. If you are not cast, it probably isn't personal - every Director sees things differently. Rejection is a regular part of performing. Always keep working on your audition pieces so to improve your chances in the future.
