Rubbing the hands together warms the lotion and makes for less of a shock on the skin.
At least, so Darling Hubby tells me when I'm rubbing lotion onto his back!
And when he's been a naughty boy, I use one hand.

Depends where I'm putting lotion. I'll use both hands to rub lotion on my legs, but one hand when I'm rubbing lotion on the other arm and vice versa. ; )

It depends. Do you want to apply lotion to one hand or both of them?

Actually I put it on the back of one hand and rub the backs of my hands together, as I don't like my palms greasy.

If I am applying it on myself, I just squirt it on me and rub it in, if I'm putting it on someone else, I warm it up by rubbing my hands together first.

It rubs the lotion on its skin, or else it gets the hose again.

I use one hand for my face and usually two for the rest of my body.
