I suppose you mean "reduced form". You divide the top and bottom by the same number until they have no factors in common. For example, 24/36 (divide the top and bottom by 12) = 2 / 3.

It depends what you mean by "standard form"

If it is a rational fraction, then it can be written as the ratio of two numbers p/q.

For example 1/3 or 6/10 or 45% (% simply means that you are dividing the number by one hundred, so that 45% = 45/100)

Next step is to make the numbers relatively prime, so that they have no common factor. This is called "simplifying" the fraction.

For example, 6 and 10 are both even, so they share a 2; we can remove that 2 from each

6/10 = 3/5

As you know, dividing something by 1 does not change its value. Also, 2/2 = 1 (so is 5/5, 734/734 and so on).

6/10 divided by 2/2 = (6/2)/(10/2) = 3/5

3 and 5 are relatively prime (they do not share a factor) so this fraction is now simplified.

45/100 = 9/20 (we removed the common factor 5).

You can also write many fraction as a decimal expansion: 3/5 = 6/10 = 0.6
45% = 45/100 = 0.45

However, some simple rational fraction do not have a finite decimal expansion, for example:
1/7 = 0.142857142857142857...
the sequence 142857 repeats forever.
So, decimal expansion is not a standard form for all rational fractions. But it is the best way to represent irrational fractions such as pi = 3.14159265358979... (which never repeats).

Egyptian scribes used a standard form where the numerator (the number above) was always a 1 (the only exception they allowed was 2/3), so that our 3/8 of above would be written 1/4 + 1/8.
45% = 1/4 + 1/5
But that is not considered a standard form today (unless you are deciphering scriptures found in pyramids...)

standard form for a fraction
if you are talking about simplifying a fraction (for example 4/6 = 2/3, then divide the numerator and denominator by a Common Divider (a number which both the numerator and denominator can be divded by). For example, for 4/6...both 4 and 6 are divisible by 2, therefore (4/2) / (6/2) = 2/3

standard form meaning scientific notation
a fraction should not be written in scientific notation (scientific notation is 2.3 x 10^4 for 23000). However, if it is necessary to write a fraction in scientific notation, almost always the exponent of 10 (called the degree) will be -1. (2/3 = .6667 = 6.667 x 10^-1)

standard form meaning equation of a line
the standard form for an equation of a line is y=mx+b. m = slope (rise / run) and b = y-intercept. if the slope of the line were a fraction (for example 2/3), that just means you rise the number on the numerator (2) and shift across the number on the denominator (3).
