My first question would be are there any other fish in the tank that he could be fighting with or that might be picking on him? Also, how long has the tank been set up and running? Ammonia problems can cause torn fins and scale loss as well. If that's not the problem, look for any red or bloody looking places on he fish. Also look for any fuzzy looking stuff and tell us where that is on the fish if you see any. Check those things and add any info you can think of and we'll be able to help you even more. There are several parasites and fungi that can cause scale loss, most of which are readily treatable, but without more information it's impossible to say which it could be.

The only reason your goldie will be loosing scales if 1 from breeding and 2 he is sick from Myxosporidiosis. The only treatment for this is to keep doing water changes and get rid of the parasites.

Also, doing water changes will help with the fin rot. your fish could also be digging beneath items and ripping his fins.
Do you have any other fish in the tank?

could be fin rot, but does not usually go to the scales. fish do lose fins as they grow, but usually one or two not a lot. my goldfish actually changed color as they grew it was crazy. i would definetly take a picture of the fish or teh fish itself to a fish store and ask them.
