Depends on which state you're in, if you're in the US. Not sure about other countries.

In CA, it's about once a year. DMV will send you renewal notice, and inside it will tell you if you need to smog your vehicle or not.

In Colorado it depends on your county. Until this year my county required cars four years and older to be smogged every other year.
