Why do you say morphine shortens the life of a person who is in hospice care?
Hospice is created to care for a dying patient. Why would you want to prolong the life when there is no quality of life?
The reason physicians prescribe morphine, fentanyl and other drugs in that class is because most of the people who are dying from a disease process have some degree of pain. Morphine is one of the most effective pain treatments we know. If we withold the morphine, does it make it better if they live a month longer but are in agony?
I'm not positive of the regulations, and I'm not sure exactly what you are asking. If you mean while the patient is at home, patients are given medications by family members all the time. If you are in the hospital, a nurse needs to administer it. If you are in a nursing home, you need to ask the director what the guidelines of the nursing home say about that situation.

My father who is in a nursing home was recovering from an illness. He kept on going downhill and we couldnt understand why. We were told he was not on morphine.(We later found out he was) We were then told he was dieing and didn't have much longer to live and to give him morphine to let him pass peacefully. He had all the side effects of morphine...we also found that out later. They didn't give him food or water for 3 days and he was severely dehydrated. He was within hours of death when a diff dr came to see him, and listened to our view what was happening, and then he immediately sent him to the hospital. He was put on an IV to get fluids back in him, taken off morphine and by the next day he was eating pureed food and talking again....just a few days after that he is eating big meals, drinking lots of fluid and doing well other than some mild confusion. I can't believe how close we came to losing him...because of too much morphine given to him over to long of a time.

A med clerk administer morphine? No, unless they want to go to prison. This is for the RN to decide the last I checked. It depresses the breathing of people who are barely clinging to life as it is. Figure that out.

Morphine does not shorten a person's life.
A pharmacist who has completed a training course can administer some shots. The ones which they can depend on the state.

You can look for anti-itch regimens like those seen over the counter. Antihistamines like promethazine are said to reduce the symptoms. Morphine can acause histamine release, which causes itching of the skin. Eye symptoms which can be due to morphine are miosis or pupil contraction (which opposite to what you experience), blurred vision, nystagmus (involuntary eye movements) & diplopia (double vision).
