A really good, cheap (but not free) source of legal forms is US Legal Forms. What you're describing, though, is NOT a "living will." A "living will" describes what measures you want taken (or not taken) if you are comatose in the hospital. Do you want them to take all available measures to resuscitate you or do you want them to let you go?

What you're describing in your additional details is simply part of your last will and testament.

I went to my local chapter of Hospice and got mine. Hospice is a wonderful organization, and the folks there are more than willing to do whatever they can, to help.
