Dying before I have kids seems to be my biggest one right now. Or generally dying before I've made a big enough mark.
Followed closely by death generally heh.

A recent dream got me thinking about this... My fear is not being able to help somone in distress.
I've taken first aid classes, have fire extingushers, first aid kit , jumper cables and tools in my car, but in my dream someone fell in the water and I couldn't do anything. I can't swim. People just ignored the person in the water and I was frantically calling 911, running up and down the pier looking for a life ring.
I have no fear of spiders, the dark, dying or living alone. In fact I love living alone, but the dream about the drowning person has been on my mind for a couple weeks.

I was about to say being alone, but I know what my biggest fear really is: becoming useless.
I have Tourettes Syndrome (no, I don't cuss at everyone and everything, but I have some of the other symptoms - and benefits, yes there are benefits; one of those is my obsessive compulsive need to know what I don't know. Hey, it's a good thing! :-)
When I hear a word or phrase used in a movie or a book, I stop, get a dictionary or thesaurus and look it up. When I get involved in a conversation where my knowledge is lacking, I look up the info and get the basics.
I like to think of myself as the go-to guy, I like to feel needed, I like to know I'm useful. I think I'd probably cease to function if I knew I couldn't be of any use to anyone anymore.

i have a fear that my son could die of something genetic and i won't know to check for it.
I have the fear that i will end up losing the love of my life because i sabatoge myself due to fear of rejection.
I'm afraid i will meet my mother or my father for the first time and want to kill them for giving me to crazy people.
I have a long list but these are the ones that bother me right now.

My biggest fears, in order, would have to be:
- Not finding the one true person I'm supposed to be with
- Death
- Spiders
- Getting cancer, going blind or deaf, or not being able to walk (at all)

I really don't fear anything. We live our lives and strive for happiness each day and then we all die. What is there to fear? The Creator chooses when and where and how we die. Who are we to argue?

Ditto, but mainly as it relates to becoming old and "infirm." As health issue keep cropping up, I'm worried that I have no one to look after me, or even to look in on me. Otherwise, I've been alone for so much of my life, it's always been part of who I am .
FTW: OK, since you mention it, wolf spiders!
