There is a girl, and she has lost a friend...I have been oblivious to all of it, and I was making jokes, like I usually did...She thinks I have been a jerk, and so all the blame's been put on me...I really am truly sorry, but I don't know how to say it...

Just tell her. It takes a big person to admit when they have been a jerk and by telling her you are truly sorry she will see that.

Since you already know you were a jerk than you need to be as honest as you can be to her. People react differently to death, so ask her to have coffee or whatever and lay it on the table. If she is indeed a good friend she will forgive you
Good luck

Walk up to her and say "I'm sorry" . You must truly mean it. If you do this and she is still thinking of you as a jerk, then move on.

Eventhough you have apologize verbally, you still should send her a card (and not an e-card) apologizing for being a jerk. (Having someone read something has a better effect than someone saying it out their mouth)...Explain to her that you didnt mean any disrespect towards her or her lost.

You have to be truly sorry to say you are truly sorry. If you are, then just take the time to tell her. She probably needs to hear it.

just say, "I shouldn't have been joking around the way I was. It was bad timing. I didnt mean for the things I said to intentionally hurt you, and I am truly sorry. "
Just be honest and straightforward.

just tell her you are extremely sorry and that everything she said about you was correct -- that you were a jerk for making up jokes and making fun of the situation and that you deserved her criticism. she might come around
