well, the receiver might be sore the first couple of times, but to get really injured, would require the giver to be really really rough. Done properly (progressively, with enough lube) you don't risk much. If it hurt, simply slow down to what feel comfortable. If you respect your body, you won't get hurt.

Yes. Top or bottom, it's risky because there is so little flexibility/stretchability there. If you;re straight, just use the front door. If you're gay, use lots and lots of lube and go reeeeealy slowly and carefully.

Use a lot of lube if you have a huge dick, use less if you have a smaller dick.
