Transmission fluid should be "flushed" (aka changed) once every 25,000 miles. Because of all the contaminants and dirt that may build up in the transmission, it could be detrimental to your car if you wait past 25k. However, checking your fluid and adding some in between that 25k, if it is needed, should be done in order to maintain a working transmission. Nothing is worse than having your car fail on you because you didn't check your fluids and add to them if low.

Transmission fluid in automatic and manual transmissions break down just like oil. On a manual you just change the oil. With automatics they must be taken to a reputable and properly equiped repair shop. They have a machine that flushes out all of the old fluid and replaces it with new. It is good to do this because just draining the fluid from the plug won't get all of it out. The torque converter has to be drained or flushed along with the rest of the transmission.

Yes. It needs to be changed at intervals of let's say every 25,000 Mls. Like all fluid, transmission oil breaks down after awhile and contaminants and particles build up withing the oil from constant operations of the coverter, valve body and gears. However, if required after weekly, forthnightly or monthly checking, fluid can be added to top up level. Note the same make and type fluid within the transmission is best required for this top up.

We have a mechanics workshop - we do Major Servicing - with this we top up the transmission oil but if we see the oil is really not looking good we recommend you take it to get the transmission serviced - its not something we do in house but we have had it done for our customers - so really both answers are right - you can top up you dont need to replace all the time like the engine oil which is emptied out in the service and replaced along with the oil filter - however every so often (cant think right now how often that is) you also need to have your automatic transmission serviced by a specialist in that field.
