All societies fail for these various, and sometimes overlapping, reasons.
1) War, either invasion or civil war.
2) Pestilence. An example would be the Byzantine Empire.
3) Drought/Famine
4) Volcanoes. See the Minoan civilization.
5) Corruptness or waste of resources, e.g., the Roman Empire.
There are some others, but you should also do your homework.

hard to say, most societies don't up and up disappear, they get assimilated into other cultures or get swallowed up by invaders (e.g. the romans), but if i had to answer,
the trojans got pretty wiped out. but later became etruscans and then romans
the picts of scotland eventually got assmilated into a different culture.
