Simply put, it means to be "self aware." "Ego" is Latin for "I am."
A healthy ego regulates the impulses of the "Id," the so called pleasure center and the "super ego," the area of executive reasoning that often indulges in schemes and manipulation.
If a person has a "weak" ego, they are at the mercy at either one or both entities. This is why some men rape women, for example, as they are completely at the mercy of their "Id" telling them to "have sex pronto!" and their "super ego" telling them "here's how...jump the next female human being passing by." A person with a "strong ego" is able to postpone pleasure until it is appropriate, for example, and go about achieving it in a way that doesn't involve the exploitation of others or breaking the law.
What many mean by "big ego" is actually Narcissism, which is endemic in our culture.

When people say "ego" they usually mean a certain personality. But there is the "id", "ego" and "superego". The ego is the neutral party that tries to fufill your desires (id) while doing the right thing (superego) at the same time.

if someone tells you you have a big ego... they're basically calling you cocky. your ego is just your perception of yourself basically.
