whenever my friend plays an online game, he registers under a fake address-example, monkeyface@blogspam.com.
Will anything happen to him?

Nope, but a few things might not work.

I sometimes don't give my email address to sites or games, because I'm worried they'll spam me. NOT ALL WEBSITES do, but I use common sense.

However, sometimes sites will require you to get a registration code from your email address; so as to prevent bot accounts. Like Yahoo! Answers, for example. You had to grab an activation code from your email address to start posting here.

It depends if this is discovered by the site he registars to... In some cases it is a Violation of their AUP and the account can be closed...
If you dont want to give out your e-mail address, goto yahoo, gmail, msn, etc and setup a seconed "Spam Account"

Not much unless he does anything illegal, as he can easily be tracked down by IP/ISP details.
