If you puncture a cylinder of carbon dioxide gas, why does the gas rush out instead of flowing or leaking out?

Because the pressure in side the cylinder is serveral thousand PSI. Normal, ambient atmospheric pressure is around 14.7 PSI.

because the pressure will want to equalize as quickly as possible. If you made a small enough hole it would probably just leak out.

Because it's under high pressure.
