The only thing that is going to get that mold out is after washing it, soak it in vinegar and water and leave it over night, so the acid in the vinegar can work. Then give it another good washing. Then it should be OK.

I would run it through a dishwasher to get the hot water on it and them bleach it and wash it again. No need to throw it away. Tupperware is very pricey.

That depends upon what you want to use it for. I've used them after cleaning out mold with no more than detergent. My wife chooses to throw them away. I suggest that you choose the level of risk with which you are comfortable. Mold might bother some more than others.

I would probably throw it away. Bleach might clean it, though.
If you scrub it, soak it in bleach, scrub again, and there's and smell or visible residue left over you should probably throw it away.

I have never tried vinegar, but that sounds like it might work, too.

As long as you bleach it, yes. My mom kept finding her Tupperware in my brothers' room, she still uses it.

I would throw it out just to be safe. New tupperware is not that expensive. :)

Personally, I'd throw it away -- or use it to store non food or dry food items.
