Data can be transferred by copying to an external HDD or by burning the data to CDs or DVDs.
If you also want to copy the OS as well as installed programs so they don't have to be reinstalled, use the clone function of Acronis True Image (Backup software).

The best way is to get a portable hard drive with a firewire connection. Firewire transfers data much faster than USB. As for your programs you can reinstall them on the new PC(if you still have the original CD) to prevent any problems from popping up.

Download and Install all your favourite programs on the new PC, they can't (and shouldn't) be copied.
Burn personal data to CD / DVD, then, transfer to the new PC.

remove the hard drive from the old PC and connects it to the new PC. Copy files from the old drive to your new drive.
Your Old PC might use IDE cables. I would recommend to remove the cables of the CD drive and connects it to the old hard drive. Once all the transfers has been completed remove the old hard drive from your new PC and connects back the power and IDE cables to your CD drive

you cant copy program you have to reinstall them hence need the cd for all the program you want to install.
to transfer you data you can burn a cd with all your data on it and copy it to your new one!
