I'd give her a bag (approach her very carefully as not to frighten her) to put her cash in and keep my gun in ready position and follow her to make sure she gets where she needs to safely. I wouldn't give her a ride since so many wrong things can happen that may land me in prison mistakenly.
Or maybe call the police and let them know of this lady.

The operative word here is HER HAND...
Not mine...I would not steal it from her or anyone...even if it was laying on the street, I would report it and take it to the police so the person who it belongs to can rightfully claim it.

hell no... I'd help her put it away in her pocket, though. If she's really so out of it that she doesn't consciously care about the money, then I'd try and contact her friends and family somehow. if that's not possible I'd take the money and give it to charity before some asshole takes it.

No, I truly wouldn't. I also tell cashiers when they give me the wrong change. I'm not embarrassed to say I'm a decent person.
